I think a lot of the time the basics don't get said and people think they don't need to state the obvious. I think we go too far in this direction. We need to state the obvious. For example, the fact is we cannot prevent these tragedies 100% of the time under the constraints we want.That is just a fact. The question is can we make optimizations? Can we do things better? What should we change? It is all a game of incentives so what levers can we pull to make things better? Then, we need trust that we are all on the same side but that's a different topic. My concern here is that we have to respect the fact that there is no acceptable way to go to 0%. There are no silver bullets. The best we can do is optimize.
